Liability Waiver and Release of Claims

I, the undersigned participant (or parent/legal guardian if under 18), acknowledge and agree that by participating in any dance classes, rehearsals, workshops, performances, or events offered by MADance Ent LLC ("the Company"), I do so at my own risk.

I understand and acknowledge that dance is a physically demanding activity that carries inherent risks, including but not limited to the risk of injury, strain, or illness. I voluntarily assume all such risks, known and unknown, even if arising from the negligence of the Company, its instructors, agents, representatives, or affiliates.

In consideration of being allowed to participate in any activities associated with MADance Ent LLC, I hereby release, waive, discharge, and covenant not to sue MADance Ent LLC, its owners, employees, instructors, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, disability, death, or loss or damage to person or property that may occur while participating in or attending activities conducted by or associated with the Company.

Photo and Media Release

I grant MADance Ent LLC the irrevocable right and permission to photograph, video record, or otherwise capture my (or my child’s) likeness, voice, and performance in connection with classes, performances, or promotional events.

I authorize MADance Ent LLC to use, reproduce, publish, and distribute such media in any format, including but not limited to promotional materials, websites, social media, advertisements, and press releases, without compensation or further approval.

I understand and agree that all such photographs and recordings are the property of MADance Ent LLC.
